| Description: | -Purpose. The purpose of this initiative is to stimulate innovative research to address inadequately answered questions about disease management in clinical practice for chronic heart failure (HF). The broader goal is to identify and improve, better implement, and disseminate clinically effective disease management tools into clinical practice to reduce HF morbidity and mortality and improve patient outcomes. -Mechanism of Support. This Funding Opportunity Announcement (FOA) will utilize the NIH Research Project Grant (R01) award mechanism. Research applications that better fit under other award mechanisms (e.g., R18, R21, SBIR/STTR) are encouraged to be submitted under those other mechanisms. -Funds Available and Anticipated Number of Awards. Because the nature and scope of the proposed research will vary from application to application, it is anticipated that the size and duration of each award will also vary.
